If you have any questions, or would like paper forms mailed to you, please contact the pharmacy at 414-385-9500. The pharmacy is open Monday through Friday, from 9am to 5pm.


For life-threatening emergencies, call 9-1-1. For urgent, but non life-threatening questions during non-business hours, you may contact our pharmacist toll-free at 855-258-8800.
